Ask before care
Ask for a Good Faith Estimate before scheduling or when arranging care. It should describe the expected charges for the planned items and services. Contact Veritas at 720-450-1690 or james@veritasmensclinic.com to request one.
For care scheduled 3–9 business days ahead, the estimate is generally due within one business day of scheduling; for 10 or more business days ahead, within three. If you request an estimate before scheduling, it is generally due within three business days. Keep a copy for your records.
If a bill is higher than the estimate
If a provider’s bill is at least $400 above that provider’s estimate, you may qualify for the federal patient-provider dispute process. Contact the clinic about billing questions, and visit CMS for current eligibility, deadlines and instructions. A published price list does not replace your individual written estimate.
