Who this policy covers
This policy describes the websites and online request forms operated by Veritas Men’s Clinic LLC at www.veritasmensclinic.com and requests.veritasmensclinic.com. It also explains how to contact us about information connected with your care.
Clinical consent forms and any separate patient privacy notices provided during care may contain additional information. This website policy does not replace those documents or limit rights you have under applicable law.
Information you share with us
- Contact requests: your name, phone number, optional email, callback preference, reason for contacting us, and communication choices.
- Medication requests: contact details, medication selections, required patient confirmations, delivery information when requested, and the status of your request.
- Care: registration details, medical history, lab results, treatment information and other information needed for your clinical care.
- Payments: information needed to process and reconcile a payment. Our online card-entry fields are provided by STAX, which processes the card details you enter. Veritas receives payment references and limited card information, such as card type and last four digits.
How we use and share information
We use information to respond to you, arrange appointments, evaluate and provide care, review medication requests, process payments, coordinate fulfillment, keep records, and protect our services from misuse.
Information may be shared with the people and providers needed for those tasks, including laboratories, pharmacies, payment processors, communication and scheduling providers, shipping services, and the services that host and support our systems. We aim to limit sharing to what the task requires. We may also disclose information when required by law or to address fraud, safety or legal obligations.
We do not sell or rent your personal information for another company’s independent marketing. Permission to contact you about a request is separate from an optional choice to receive clinic news.
Calls, email and text messages
When you ask us to contact you, we use your details to respond to that request. Care-related communication may also include appointment, follow-up, billing and medication updates. Tell the clinic if you have a preferred or safer way to be contacted.
If you choose the optional clinic-news email checkbox, we may email clinic updates and hormone-health education. You can decline those emails without affecting care and unsubscribe later using the message’s unsubscribe option or by contacting us.
For eligible text messages, use the opt-out instructions in the message or contact the clinic. Let us know if you want to change how you receive care-related messages so we can discuss another way to reach you.
Ordinary email may not be appropriate for urgent or highly sensitive information. Keep symptoms, lab results, medical records and card details out of public contact messages. Ask the clinic how to share them appropriately. For a medical emergency, call 911.
Website analytics and browser storage
On selected informational pages, we offer optional Google Analytics measurement of visits and general clinic phone- or email-link clicks. The analytics tag stays off until you allow it. Care-request forms, patient services and staff pages are excluded from this measurement. Your choice does not affect access to the website or care.
Our optional analytics controls respect Global Privacy Control and Do Not Track signals. Use Privacy choices on a page that offers analytics to allow, decline or withdraw permission. A link click does not confirm that a call connected or an email was sent.
The clinic separately keeps received requests and staff follow-up records to respond to you and manage its work. These operational records are different from optional website analytics.
If you choose Remember for 30 days on the medication form, your name, phone number and email can be saved in that browser. Remembering starts off. Avoid enabling it on shared devices. The Cookies and browser storage page explains these controls and how to remove saved details.
Keeping information safe and retaining records
We take practical steps to protect information and limit access to people and services that need it for their work. No website, device or method of communication can guarantee complete security. Please tell us promptly if you think information connected with the clinic has been exposed or used incorrectly.
We retain information for care, follow-up, payment, recordkeeping, security and applicable legal requirements. How long it is needed depends on the type of record and why it was collected. Removing saved details from your browser does not remove a request or care record already received by the clinic.
Questions, corrections and privacy requests
Contact us to ask about information we hold, request a copy or correction, discuss removal where permitted, change communication preferences, or raise a privacy concern. We may need to verify your identity before sharing or changing information. Some medical, payment and other records must be retained; we will explain relevant limits when responding.
Our online request forms are intended for adults seeking care. If you are contacting us on someone else’s behalf, or have a concern about a child’s information, call the clinic so we can discuss the appropriate next step.
